What goes in each field

Date
The day the instruction happened. Log as you go -- a date written a month later is the part a reviewer can most easily doubt.
Subjects / Activities
What you actually covered. Specific beats tidy: "fractions, word problems" is worth more than "math".
Hours
Time spent, in hours or fractions of one. Count instruction, reading, and the field trip; the sheet has room in Notes for anything you want to justify later.
Notes
Anything you would want to explain a year from now -- an illness, a co-op day, a curriculum change mid-year.

Print one sheet per month
Twelve sheets covers a school year with room to spare. Keep them in one folder rather than loose.

Fill it in the same day
The log is worth what its dates are worth. Two minutes at the end of the day beats an hour of remembering in June.

Total the month and compare it to the target
Add the Hours column at the end of each month and hold it against Louisiana's 180 days a year. Catching a shortfall in November is a scheduling change; catching it in May is not.

The log is the piece that answers "what did you actually do" -- keep it with your filings and work samples.

Does Louisiana require me to log hours?
Louisiana requires 180 days a year. The statute sets the amount rather than the format, so how you record it is up to you -- but you need to be able to show you met it.
Do I have to send this to anyone?
Not the log itself. Louisiana asks for a report on a annual basis, and the log is the evidence behind what that report says rather than an attachment to it.
Is a handwritten log good enough?
Yes. Nothing in any state's rules requires software. What matters is that the record is contemporaneous and specific enough to show what was taught and when.
